GUIDELINES TO FILL THE PRESIDENT'S ROTATING THOPHY  PROFORMA

GENERAL INFORMATION : Appropriate information should be given as regards Serial No.1 to 3.

  1. Activities of the Society :  Number of Managing Committee Meetings held minimum 4 Managing Committee Meetings should be held in the year, every 3rd month to deliberate the agenda for the FOGSI Managing Committee Meeting, i.e. the Managing Committee should be held 2 to 3 weeks before the FOGSI Managing Committee Meeting and relevant deliberations should be conveyed to FOGSI Office, either through their representatives or by correspondence.  This will help the FOGSI Office to get the views of the Member bodies at the time of the Managing Committee Meetings.
  2. Number of General Body Meetings Held :  There should be at least 3 General Body Meetings of the Society.  First should be 2 to 3 months before the annual congress of FOGSI for putting new proposals etc. to the General Body, second meeting should be immediately after the receipt of the Secretary General's circular and third meeting for sending the proposals for office bearers of FOGSI.
  3. Number of New Members Enrolled During the Year :  Only the number of new members should be written. (The number of old members should not be included in it).
  4. Number of Clinical or Scientific Meetings :  Minimum 6 meetings are required to be held to qualify for the competition.
  5. Number of Guest Speakers Invited & Orations Held During the Year :  While stating this, attention should be given to the following points:
    1. The stating(s) of the Guest lecture(s), Oration(s).
    2. Name(s) of guest speakers(s) invited.
    3. Whether Active member(s) of the Federation has/have received any encouragement.
    4. Whether the guest speaker is a member of any member body of FOGSI other than the host society  (intention of establishing fraternity with other societies which itself is an aim of FOGSI).
  6. Details of Symposia :  Minimum 2 are required for qualifying and more for contesting.  Reports in brief, with programme should be funshed.
  7. Continuing Medical Education Programme :  One for qualifying and minimum two or more for contesting.
  8. Incentives for Research or Academic Activities of the Society :  Please state whether any member of your society has get felicitations, prizes or awards from FOGSI or Govt./Local Body other than host society     for research Doctor.
  9. Details of Involvement :  Details of involvement in Urban or Rural community activities within and outside the jurisdiction of the society or any other socio-medical activities, like adopting village etc.
  10. Annual Convention :  Annual Convention or involvement in regional conferences or state conferences, even in collaboration with I.M.A. Conference of with other faculties, will be taken for consideration.
  11. Refresher Courses :  Details of refresher course, training course and other details like no of participants attendants should be given.
  12. Correspondence and Submission to FOGSI :  Correspondence and submission of periodic reports to FOGSI Office should be given.
  13. Attendance of Representative(s) to FOGSI Managing Committee : Attendance of representative(s) to FOGSI Managing Committee Meeting or submission of relevant reports as pointed above in Item No.1.  This will amount to the attendance of representatives of the member bodies to Managing Committee.
  14. Attendance of Representative(s) at General Body Meeting :  Reports of attendance at the General Body Meeting of FOGSI should be given.
  15. Details of Participation in FOGSI Committees :  Involvement of the society in FOGSI activities like formation of sub committees in the same pattern as FOGSI Committees.  Activities of these committees should be entioned.
  16. Number of Members Attending Previous FOGSI Conferences :  The Statement should show the number of members of the member body who have attended the FOGSI conference in last 4 years.
  17. Regularity of  Payment Annual dues to FOGSI :  The date when the FOGSI dues of the society are paid.
  18. Contributions to FOGSI Building Fund :  Any activity to raise the Building Fund and the amount of money paid towards building fund should be mentioned.
  19. Any other Relevant Activities :  Any handicaps or hardships under which the society is functioning.
